2011-2012 FAFSA Worksheet: Students requesting financial aid must complete a Free Application for Federal Student Aid (FAFSA). This worksheet should be used as a guide to help you file online.

Drug-Related Conviction Worksheet: If you have been convicted of possessing or selling illegal drugs, complete this worksheet to determine if you are eligible for federal student aid.

Request for Independence: Independence for federal financial aid purposes may be granted where there is an involuntary separation from the family or if extraordinary circumstances, such as an abusive family environment, exist. An involuntary dissolution occurs when a student is separated from the family for reasons other than his/her or the parents’ own choice. Documentation is required and must be attached to this request.

APTS Application: The Aid for Part-Time Study (APTS) program provides grant assistance for eligible part-time students who are enrolled in approved undergraduate programs. College financial aid administrators select potential recipients from eligible students and decide who will receive APTS awards based on the funds they have available and individual student need.

Verification Worksheet for Dependent Students: If your application was selected for verification and you are a dependent student, complete the Dependent Verification Worksheet.

Verification Worksheet for Independent Students: If your application was selected for verification, and you are an independent student, complete the Verification Worksheet for Independent Students.

Work Study Request: To request a work study award or an increase to current award, please print this form and bring it to the financial aid office.

IRS Tax Return Transcript Online Request: If selected for verification, you will have to forward your tax return transcript to the College.
